Bakset Challenge Form

Organizations desiring to enter a gift basket into the "Basket Challenge" must complete the contest entry form below by August 26, 2009. The baskets must be delivered no later than September 9, 2009 to the PCCH office. This year, there are two categories in which to compete: 1) "Most Appealing Basket" - that is, the basket that looks the best, and 2) "Most Rewarding Basket" - that is, the basket the brings in the highest bid. The winning organizations will receive prizes.
